In order to complete the process of
enrolling your child(ren), you will need to present the following:

A completed application form. (Download the
form above) Additional forms such as emergency contact forms, permission
forms, etc., will be completed at Lutheran Day.

A completed Physical which includes a TB
Test and Results, a Lead Test and Results and a Diabetes Screening. Your
physician will have the appropriate form.

2" X 2" photo of your child taken within 1
month prior to enrollment

Your child's birth certificate
-
A copy of your state ID or Driver's License
-
A copy of your most recent IRS Form 1040,
1040A, or 1040EZ (to determine your weekly fee)

$25.00 Registration Fee *

Security Deposit (determined by your weekly
fee) *
-
$10.00 to purchase your child a Lutheran
Day T-shirt for field trips.*
* Please bring a separate check or money
order for the registration fee, security deposit, and T-shirt. We do not
accept cash payment for registration, security deposit or regular tuition
payments.
